The mechanism of action of paracetamol is complex and involves inhibition of prostaglandins to exert its analgesic effect.1 The antagonism of adenosine receptors and inhibition of cyclo-oxygenase activity at some sites may explain caffeine’s adjuvant properties.2,3

First discovered in 350 A.D., caffeine was extracted as an active compound in 1819.
Caffeine basics
It is the most commonly used legal psychoactive drug and is a chemical relative of theophylline (used in treatment of asthma).2,4 Known for some of its effects such as increasing metabolic activity and heart rate, alertness, wakefulness and concentration, for several decades it has been combined with other medications as an analgesic adjuvant.2,4
